In relation to the TWSCS application form, due to GDPR guidelines you MUST NOT input employee personal data on the form i.e. employee name / initials.
When completing the employee ID information section of the form, details must be input in the following format (as per the details in the guidance document): DCYA Reference Number and an underscore (_) followed by a 3 digit unique identifier for each employee e.g. 18SO12345_123
Any service provider who has not input details in the above format, must edit the form by deleting each incorrect employee record, add it again in the correct format, and resubmit the form.

Note: Payments can only be made on applications which are completed and submitted in line with the guidance document.
